Request a certificate for any sacrament done at St. Justin Martyr Church and Sacred Heart Mission.
Sacraments you may request a certificate for are:
-Baptism
-First Communion
-Confirmation
-Marriage
Please note that we do not keep records of sacraments done at other parishes. Each parish keeps its own record. An excellent place to start in locating a parish is the Diocese/Archdiocese website. Most Diocese/Archdioceses have a directory for the parishes in their Diocese.
The person requesting the certificate must be the person who did the sacrament or a parent of a minor.
Please fill out the Certificate Request form and subit online or you may drop it off at the parish office.
A fee of $10 for each certificate.
Accepted forms of payment are debit, credit card, or check (payable to St. Justin Martyr Church).
Why do I need to keep certificates on file?
Parents are encouraged to keep the Baptism certificate of their child because just like a birth certificate it is a document that proves the Baptism of the child. To enroll in faith formation classes for First Holy Communion and Confirmation, the parishes will ask for the Baptism Certificate and a First Communion certificate.
Marriage preparation documents include obtaining a newly issued Baptism certificate and either the original First Communion and Confirmation certificates or newly issued certificates.
Can anyone request a certificate?
Only the person who did the sacrament or, in the case of minors, the parent or legal guardian can request a certificate of their child. We ask for verification of personal information to prevent stolen identities, which though rare, have happened in sacramental records.
Why is there a fee?
The parish as a nonprofit organization relies on donations and fees to keep it open. The parish office has employees to ensure parishioners and the community are served as best as possible and are dedicated to running the parish office, something that cannot be done just with volunteers. Those employees must be compensated for their time as required by law.
